This form is for registering schools and home-schooled children to attend the event, it contains important information about our sessions

Please read this introduction carefully before proceeding.

• You are responsible for transportation from school to the library.
• You agree to distribute photography consent forms to students’ parents or guardians and return completed forms to Joondalup Libraries staff by the date requested.
• Register each class separately using a new form.
• The program is very popular, and while we strive to match preferences, it may not be possible for everyone.
• Registration closes Monday 26 May. Initial offers will be made by Monday 9 June.
