Please note that all partners are required to retain and provide evidence of current Public Liability Insurance to a minimum value of $20 million.

Collection Notice

The City of Joondalup collects the personal information on this form to assess and manage your expression of interest to perform, present, or participate in City cultural events, programs, and activations.

Providing this information is voluntary, and by submitting this form you consent to the City using and disclosing your information for this purpose and directly related purposes. This may include disclosure to relevant City employees and contractors involved in the planning, assessment, administration, and delivery of cultural events and programs. Information may also be provided to event organisers, venues, suppliers, insurers, or other parties engaged by the City where necessary for event planning, administration, risk management, or operational purposes.

If you do not provide the requested information, the City may be unable to assess your suitability or contact you regarding relevant opportunities.

You may withdraw your consent by contacting the City in writing at info@joondalup.wa.gov.au or by calling (08) 9400 4000. If you withdraw consent, the City may be unable to continue processing your application. Withdrawal will not affect any use or disclosure that has already occurred before the withdrawal was received.
