Collection Notice

The City of Joondalup collects the personal information on this form for the purpose of assisting with the submission, update, or correction of information for the Community Directory listing, including storage, administrative and management operation of the Community Directory service.

The information collected may include organisation details, contact names, contact phone numbers, email addresses, postal or meeting addresses, website details, and other information relevant to the directory listing.

The Community Directory is maintained by Data Diction. Information submitted through this form will be disclosed to Data Diction so that the relevant listing can be assessed, updated, maintained and, where appropriate, published in the publicly accessible Community Directory.

Providing this information is voluntary. By submitting this form, you consent to the City collecting, using, and disclosing your personal information for the stated purpose and directly related purposes, including disclosure to Data Diction and publication in the Community Directory, where appropriate.

If you do not provide the requested information, the City may be unable to process your submission.

You may withdraw your consent by contacting the City in writing at info@joondalup.wa.gov.au or by calling (08) 9400 4000. Withdrawal will not affect any use or disclosure that has already occurred before the withdrawal was received.
